Heat movement, almost always. A terrace can swing forty degrees between a February night and an August afternoon, and the tiles expand and contract against a slab that moves at a different rate. If there is no movement joint, or the tiles were fixed with a rigid adhesive straight onto an old surface, something has to give, and the tile is what gives. Water getting under a lifted tile then finishes the job in winter.
It means the adhesive is no longer holding the tile to the surface underneath, whether it was never fully bedded or it has released since. A hollow tile is not an emergency, but it will crack the first time something heavy lands on it, and water reaching the gap will spread the problem to its neighbours. Tap across the floor with a coin and you will hear where the sound changes.
Removing is the safer choice on a floor and often the only choice in a bathroom, because tiling over raises the level at doors and against the shower tray. Tiling over can work on a sound wall with a suitable primer and adhesive, and it saves a lot of dust and waste. The decision comes down to whether the existing tiles are firmly stuck, which is a five minute check on site.
